Release of Liability and Assumption of Risk

I, the undersigned participant and/or parent or legal guardian of the minor child(ren) listed above (collectively, “Participants”), in consideration for being permitted to attend or participate in any activities, events, gatherings, or programs hosted, sponsored, or coordinated by Cypress Homeschool Association (“CHA”), whether as a member or non-member, hereby agree as follows:

  1. Assumption of Risk: I acknowledge that participation in CHA activities may involve inherent risks of injury, illness, property damage, or other harm, and I voluntarily assume all such risks for myself and the Participant(s).

  2. Release of Liability: I hereby fully and forever release, discharge, and waive any and all claims, demands, rights, or causes of action against CHA, its officers, directors, volunteers, members, representatives, and agents, arising from or related to any injury, illness, including death, property damage, or other loss suffered by any Participant, whether arising from ordinary negligence or any other act or omission of CHA.

  3. Hold Harmless and Indemnification: I agree to hold harmless, indemnify, and defend CHA and its representatives from any claims brought by or on behalf of any Participant, or by any third party, arising out of or related to the Participant’s involvement in CHA activities.

  4. Medical Authorization: In the event of injury or medical emergency involving a minor Participant, I authorize CHA representatives to obtain emergency medical treatment deemed necessary, and I assume full financial responsibility for such treatment.

  5. Compliance With Rules: I agree that all Participants under my care will follow CHA rules, instructions, and safety guidelines. I acknowledge that failure to comply may result in removal from an activity or event.

  6. Photography and Media Release: I grant CHA permission to photograph or record Participants during CHA activities and to use such materials for CHA publications, promotional materials, and members-only digital platforms. I understand that Participants’ names will not be publicly associated with media without additional consent.

  7. Understanding and Voluntary Agreement: I certify that I am at least 18 years old, have read and understand this release, and am voluntarily signing it on my own behalf and/or as the legal guardian of the minor Participant(s) listed.

This Release shall remain valid for all current and future participation in CHA activities unless revoked in writing.
